- Job Overview:
The Enrollment Nurse is responsible for completing the UAS assessment, reviewing the NYIA assessment, creation of the time tasking tool for the potential member, and development of the Person-Centered Service Plan (PCSP). The Enrollment nurse also conducts PCSP meeting with the potential member and obtains the enrollment documentation to ensure potential member enrolls into the Plan- Conducts UAS-NY assessment visits for potential plan members.
- Conducts assessment of newly referred members, assesses overall health needs and eligibility for enrollment.
- Provides member and family teaching at initial visits
- Post-visit communication with intake/enrollment related to member’s eligibility status. Post visit communication with members of care management and care planning teams.
- Initiates development and coordination of a safe plan of care with member/family to meet all members’ needs; coordinates with rest of care management team.
- Documentation of UAS-NY and visit notes including progress note and PCSP.
- Communication of members’ needs and identified care plan to members’ care manager
- Attends professional and required in-service programs and staff meeting
- Participates in the agency QA Program, as requested.
- Is available to cover shifts for other nurses as necessary and reasonably able
- Performs assessments and completes documentation within the time frames set forth by the Agency
- Participates in mandatory staff meetings
- Meets productivity requirements as set forth by Agency.
- Travels as required as set forth by Agency.

Specific Knowledge, Skills, and Abilities:- Knowledge of NYS requirements specific to MLTC enrollment and marketing guidelines preferred.
- Demonstrates excellent communication, negotiation, organizational and public relations skills.
Essential Responsibilities:
- Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job
responsibilities.
- Proficiency in the use of office equipment, Microsoft Office Suite and Electronic Health
Records system.
