Bluegrass Kidney Consultant’s has been providing the best in renal care for over 15 years in Louisville and Southern Indiana and we continue to grow. We are seeking a dedicated and experienced Registered Nurse Renal Case Manager to join our multidisciplinary healthcare team. The RN Renal Case Manager will play a pivotal role in coordinating care for patients with kidney-related health issues, including chronic kidney disease (CKD), end-stage renal disease (ESRD), and those undergoing dialysis treatment. This position requires exceptional clinical skills, strong communication abilities, and a commitment to providing patient-centered care.

Key Responsibilities:

  • Patient Assessment: Conduct comprehensive assessments of patients’ medical history, current health status, and psychosocial factors affecting health outcomes.
  • Care Coordination: Coordinate and manage the delivery of care for patients across various settings, ensuring seamless transitions and continuity of care focused on renal health.
  • Education and Support: Provide education to patients and their families regarding renal disease, treatment options, medication management, diet, and lifestyle modifications to optimize health outcomes and to reduce unnecessary hospital readmissions.
  • Treatment Plan Development: Collaborate with nephrologists, nurse practitioners and the entire care team to develop individualized care plans based on clinical assessments and patient needs.
  • Monitoring and Follow-Up: Monitor patient progress, response to treatments, and adherence to care plans. Adjust care plans as necessary and provide appropriate follow-up to address any emerging issues.
  • Advocacy: Serve as an advocate for patients, ensuring they understand their rights, treatment options, and access to community resources and support services.
  • Documentation: Maintain accurate and timely documentation of patient assessments, care plans, interventions, and outcomes in accordance with regulatory standards and organizational policies.
  • Quality Improvement: Participate in quality improvement initiatives, clinical audits, and evidence-based practice projects to enhance renal care services and improve patient outcomes.

Qualifications:

  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) considered with relevant experience.
  • Current Registered Nurse (RN) licensure in [State/Region] required.
  • Minimum of [2 years] of nursing experience, with at least [1 year] in renal care or related fields (dialysis, transplantation, nephrology) and a minimum of 1 year in case management or disease management.
  • Certification in Nursing Case Management (CCM) or nephrology nursing (CNN) preferred but not required.
