Job Description

We are seeking an experienced and dynamic Clinical Nurse Trainer/Education Specialist with a focus on care management to join our team. This role will be pivotal in developing, implementing, and evaluating educational programs and training related to care management practices. The ideal candidate will bring a wealth of knowledge in clinical care management, along with a strong background in nursing, pharmacy, or advanced practice nursing.

WHAT YOU’LL BE DOING

  • Creating and refining training programs and educational materials specifically for care management for RN and Medical Assistant (Healthguide™) level of licensure/certification.
  • Ensuring that the content is evidence-based, up-to-date, and tailored to the needs of clinical staff and healthcare provider.
  • Leading training sessions, workshops, and seminars on care management concepts and practices. Utilizing diverse teaching methods and technologies to enhance the learning experience and effectiveness.
  • Providing ongoing expert guidance on care management processes, in accordance with all current applicable federal, state, and local standards, guidelines, and regulations.
  • Conducting on going education to clinically licensed and certified staff members to update knowledge, skills, and attitudes necessary to perform assigned duties safely and effectively.
  • Working with staff to understand deployed predictive models and how to most effectively impact patient outcomes.
  • Evaluating the competencies of staff in care management through assessments and skills evaluations. Identifying training needs and areas for improvement and provide targeted support.
  • Collaborating with quality improvement teams to align training programs with organizational goals for exceeding quality metrics and regulatory requirements.
  • Contributing to initiatives aimed at enhancing patient outcomes and care processes.
  • Developing and maintaining educational resources, including manuals, guidelines, and online modules related to care management and ensuring these resources are current and accessible.
  • Ensuring that all educational activities and care management practices adhere to relevant regulations, standards, and best practices.
  • Staying updated on changes in care management regulations and industry standards.


Qualifications


WHAT YOU’LL NEED TO HAVE

  • Registered Nurse (RN) with a bachelor’s degree in nursing (BSN) or higher, OR Doctor of Pharmacy (PharmD), OR Nurse Practitioner (NP) with an advanced degree (MSN, DNP, or equivalent).
  • Current RN license.
  • Certification in Care Management (CCM) or Clinical Case Management (CCMC) preferred.
  • Advanced Practice Certification (for Nurse Practitioners) or relevant board certification (for PharmD) is a plus.
  • Minimum of 3 years of clinical experience, plus
  • Minimum 2 years care management experience
  • Minimum 2 years proven experience as clinical educator developing resources]
  • Experience developing and delivering educational content related to care
  • In-depth knowledge of care management processes, including care coordination, patient advocacy, and outcome measurement.
  • Excellent communication and presentation skills.
  • Proficiency in educational technology and learning management systems.
  • Strong organizational and time-management skills.
  • Ability to engage and motivate diverse audiences.

Additional Information

The salary range for this role is between $85,000.00 and $90,000.00 per year

COMPENSATION:

The listed compensation range listed is paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
