Organization Description: Transitions Clinic Network (TCN) supports a national network of clinics that support individuals with chronic health conditions recently released from incarceration. Founded on the idea that the people closest to the problem are also closest to the solution, each clinic that adopts the TCN program employs a community health worker (CHW) with a history of incarceration as part of the primary care team. Developed in San Francisco in 2006, TCN has supported sites in 14 states and Puerto Rico in implementing the TCN program. In California, TCN has 21 sites in 14 counties.

TCN provides technical assistance and training to health systems including providers and CHWs, creates a forum for sharing best practices, promotes quality improvement and supports program evaluation. TCN strives to amplify the voice of those impacted by incarceration and mitigate some of the long-lasting harms of mass incarceration.

To increase continuity of care from state prisons in California, TCN started the California Reentry Hub, which consists of two main entry points for returning community members with complex physical and behavioral health needs in California: an online referral platform, used by prison medical staff to refer patients to TCN programs for post-release care, and a reentry healthcare hotline that serves currently incarcerated and recently released individuals and is staffed by TCN CHWs across the state.

Position Summary: The Reentry Hub Nurse Manager will oversee the operations and further development of the California Reentry Hub. This position will work closely with state prison system medical staff, TCN community clinic staff, and other external stakeholders to develop best practices and workflows for referrals to improve care continuity. The Hub Nurse Manager will provide oversight and clinical supervision to the referral coordinator and program assistant who maintain incoming referrals and create discharge plans and the CHW Hotline Coordinator who is responsible for managing the CHW staffed reentry healthcare hotline. The Hub Nurse Manager will provide oversight of program data collection and manage grants/contracts that are funding the Hub, ensuring all deliverables are met.

Primary Duties for this position include:

Program Management

  • Oversee all operations of the California Reentry Health Hub, including the online HIPAA-compliant referral system and the reentry health hotline.
  • Manage the referral process for individuals transitioning from state prisons, ensuring that their care needs are assessed and addressed effectively.

Supervision and Support

  • Supervise the referral coordinator and program assistant in managing referrals and discharge planning.
  • Oversee the CHW Hotline Coordinator, ensuring effective hotline management, coverage, and staff support

Clinical Oversight

  • Provide clinical review and consultation on complex patient cases.
  • Coordinate and offer pre-release education, harm reduction, and health education both in-person at prisons and through written materials.

Stakeholder Engagement

  • Establish and maintain relationships with prison medical staff, safety-net clinics, and other community partners.
  • Work with these stakeholders to refine referral workflows and improve care continuity.

Quality Improvement

  • Develop and disseminate best practices for pre-release planning.
  • Collaborate on quality improvement, evaluation, and research related to the Reentry Hub.

Data Management and Reporting

  • Maintain data systems and oversee the evaluation of Hub activities.
  • Manage grants and contracts, ensuring compliance and timely delivery of deliverables.

Policy and Procedure Development

  • Participate in the development of policies and procedures related to the Hub and TCN Care Coordination Activities.

Additional Responsibilities

  • Perform other duties as assigned to support the goals and operations of TCN.

Required Qualifications:

  • Master’s Degree or higher in nursing.
  • Active California licensure in nursing.
  • Experience providing clinical care coordination and direct services to marginalized or complex populations in a healthcare agency or community-based organization.
  • Demonstrated experience in supervising and managing teams.
  • Proven experience in grant writing, data analysis, program oversight, and system building.
  • Excellent organizational, documentation, and communication skills.
  • Detail-oriented and self-directed with the ability to prioritize and manage multiple tasks.
  • Ability to work effectively with a multi-disciplinary team.
  • Proficiency in Microsoft Excel and familiarity with web-based survey tools (e.g., Qualtrics or SurveyMonkey).
  • Strong commitment to improving health and wellness and amplifying the voices of individuals and communities impacted by mass incarceration.

Preferred Qualifications:

  • Experience working in a community-based healthcare agency or managing programs/research related to healthcare.
  • Experience working with individuals impacted by the criminal legal system and developing relationships with criminal legal entities (e.g., prisons, jails, probation, parole).
  • Skills in crisis management, including conflict resolution and addressing suicidal ideation.
  • Familiarity with social service and reentry organizations.
  • Understanding of Medicaid policies and ongoing reforms.

Travel: Position may require some travel (~1 day per month)

Salary Range: $130,000 to $168,000. Salary commensurate with experience.

Note: The Reentry Hub Nurse Program Manager will be employed by the fiscal sponsor of TCN, the San Francisco Public Health Foundation.
