About the Role
You will work in direct partnership with our physician team to deliver an exceptional, best-in-class clinical experience focused on transforming patients’ health through nutrition, exercise, sleep optimization, and behavioral change.
As Health Coach, you will be your patients’ dedicated guide in achieving their wellness goals. You’ll craft and oversee personalized health strategies, diving deep into their nutrition patterns, physical activity, sleep habits, stress management, and emotional wellbeing. With a focus on behavioral change, you’ll empower patients to build healthy habits that last, supporting them in overcoming challenges on their path to wellness.
Your impact will be measured in the transformative health outcomes of your patients. You’ll masterfully assess their current health status, collaborate on achievable goals, and provide the daily support and accountability that drives real change. While managing incoming questions and proactively supporting patient care, you’ll serve as the vital bridge between patients and our expert healthcare team. You’ll leverage your broad understanding of health fundamentals while knowing precisely when to engage our specialists for deeper expertise.
This is a pioneering program. You will drive innovation in patient experience, developing new approaches to help patients succeed in their health journeys. You’ll create systems that enhance accountability, celebrate victories, and ensure consistent progress toward wellness goals.
Together with your team, you will build and refine the processes that define excellence in patient care. We believe in the power of peer collaboration and continuous improvement. You’ll both give and receive feedback in ways that empower those who, like you, are passionate about revolutionizing healthcare delivery through lifestyle medicine.
Your success will be reflected in the trust you build, the sustainable health transformations you guide, and the lasting impact you create in your patients’ lives.
