Duffy is looking for a nurse who currently works in outpatient primary care, preferably community health, who has experience providing education to at risk patients and proficient in Epic, handling Rx refills, Prior Authorizations and doing medication reconciliation. The ideal candidate for this position has strong oral communication skills and is able to work independently to maintain organizational productivity metrics.

Statement of Duties :

Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively using a team-based approach. This position is responsible for providing skilled nursing care remotely as patient contact occurs mainly on the phone and via email. Registered Nurse experience preferably in community health, primary care with administration related responsibilities including Rx refills, Prior Authorizations, Phone Triage, Telehealth. Ideal candidates would have had experience with phone triage, prescription refills, submitting Prior Authorizations to insurance payers, and providing patient education. Experience with Epic and MassHealth are preferred. Professional demeanor working with at risk populations, providing non-judgmental, helpful service is essential.

Position Functions:

The essential functions or duties listed below are intended only as illustration of the various types of work that may be performed. The omission of specific statements of duties does not exclude them from the position if work is similar, related, or a logical assignment to the position.

1. Phone Triage: Reviewing voicemail and answering calls on the Nurse line voicemail. The nurse will review, document, and return calls in a timely manner, to assist patients. Calls may involve determining if patients will need an appointment, and scheduling appointments for providers or nurse visits. The ability to triage and determine if patient problem needs evaluation at ED versus provider appointment is essential. Calls may involve reviewing medication, providing education, reviewing care plans after discharge, and providing education. The nurse will also route and take action on calls from partner services, including Home Health agencies, medical supply, pharmacies and other health services. The ability to document and send encounter messages to providers in Epic is required.

2. Medication refills – Nurse will monitor in baskets for E-prescribing, review medication requests, provide proper documentation, MassPat review, if necessary, document and pend refills, sending them to the prescriber. May involve calling or sending MyChart messages to patients for more information, providing education, assessment and to follow up that refills have been sent.

3. Prior Authorizations – some refills will need a PA, nurse will complete PA and submit documentation, as well as creating an encounter in chart of this activity. Nurses may also need to assist with documentation for filling durable medical equipment, working with medical suppliers.

4. Patient Advice – monitoring MyChart messages, responding or forwarding as needed.

5. Results – calling patients to inform of recent lab results, and documenting in Epic. Medical Department

6. Other administrative responsibilities may include creating letters for patients, scheduling appointments, calling patients for hospital follow-up appointments, and conducting medication reconciliation, health question screenings and serving as a backup for telehealth services, providing support to providers conducting telehealth appointments.

7. The ideal candidate would be able to prioritize tasks, stay organized to follow up on outstanding tasks and maintain consistent productivity with assignments.

8. Participates in training new staff on required subjects such as bloodborne illnesses.

9. Maintains current licensure and certification for the position; participates in training and continuing education to stay abreast of practices and trends.

10. Full time 40-hour work week. M-F 8am-430pm with a 30-minute break. Schedule may include occasional 4-hour telehealth coverage on Saturday. Candidate would work onsite for 2 weeks of training before remote assignment begins.

Education and Experience :

Massachusetts RN required – BSN preferred.

Minimum of 2 years, preferably a candidate that has at least 3-5 years’ experience in a medical office setting, primary care, Community Health desired.

Experience with Epic preferred, comfortable working with Electronic Health Records.

Licenses and Certifications Required Must be licensed as a Registered Nurse in the state of MA, and successfully complete the credentialing process.

Knowledge, Skills and Abilities:

A candidate for this position should have knowledge of: Nursing theory and practice Rules, regulations and policies of the health care facility HIPAA regulations Trauma-informed care.

Pay: $78,000.00 – $83,000.00 per year
