We are seeking a RN for After Hours coordination of our home health division.
This is a full time, work-from-home position, anywhere in the United States.
Requires:
– RN in Virginia or a compact state
– Must have 2+ years of Homecare Homebase experience
– Must have home health clinical manager experience
– Expert in home health intake process
– Expert in Medicare and JCAHO home health regulations
Work Schedule:
– Mon-Fri at your desk 5pm-8pm, then clinical triage for calls until 9am
– Every other Sat & Sun at your desk 9am-5pm, then clinical triage calls until 9am
– Full day coverage on half of the 8 company holidays
Duties:
– Clinically triage incoming patient, family and clinician calls, texts and emails
– Dispatch clinicians after hours to resolve patient care issues to prevent re-hospitalizations
– Monitor e-charge systems to accept and fully process new patient referrals
– Schedule admissions, discuss with clinicians and adjust schedules for the next day
– Payor eligibility and obtain initial authorizations
– Review referrals for appropriateness for home care and homebound status
– Introduce agency to new patients for admits that occur after hours
– Troubleshoot clinician staffing issues that occur after hours

Pay: $65,000.00 per year
