How You’ll Make a Difference:
Arkos Health is seeking an Emergency Department Care Coordinator. The Emergency Department Care Coordinator is a full-time, telephonic position covering normal business hours during the week to provide support and assist in bringing members to the most appropriate level of care necessary by assisting ER staff in coordinating dispositions to home and skilled nursing facilities and potentially avoid unnecessary in-patient hospitalizations.
Please note: Must reside and be fully authorized to work in the US.
**We are seeking remote applicants, preferably in Arizona, Texas, New Mexico, Nevada, or any state except California, Colorado, New Jersey, New York, Oregon, Massachusetts and Washington.
Essential Duties and Responsibilities:
- Monitor and coordinate HIE and ER alert notifications, covering the days, evening and weekend hours
- Appropriately identify member needs through in-coming HIE alert and chief complaint notifications based on available Arkos programs and solutions
- Contacts and coordinates with ER staff, offer appropriate USHS services (acute NP visits, ICM, TCM, Home Health, DME, etc.) based on data collection from ER
- Coordinates ER to SNF admissions, facilitating coordination to available beds and acceptance from SNF admission staff
- Monitor and clear alerts from HIE and document appropriately in member chart
- Accurately chart and notify internal staff through the referral portal for member needs, status and placement
Qualifications and Skills:
- Medical Professional with either EMT, Medic, LPN or MA certificate/License
- Must be able to interact with patients, staff and physicians in a professional manner
- Ability to maintain confidentiality and HIPAA compliance
- Ability to multi-task and prioritize in high stress environment
- Ability to make decisions by using logic to identify key facts, explore alternatives and propose quality member solutions
- Ability to find, gather and collect data
