Job Summary:

The Heart Failure Nurse Practitioner (NP) will provide specialty level heart failure care to a defined panel of patients using advanced Remote Patient Monitoring (RPM) technology. Applying clinical expertise and knowledge of heart failure management, the Heart Failure NP will use decision-making skills to deliver remote care management. Once a treatment plan is initiated, the follow-up outreach, education and titration will be delivered collaboratively with a Brick and Mortar Heart Failure clinic by a Heart Failure NP and Cardiologist with the assistance of the Circadian Care Team. This care team is composed of Registered Nurse Care Managers (RNs), and patient coordinators, all of whom will work simultaneously to move patients to their targeted health goals. The Heart Failure NP will serve as a clinical resource and educator for the team with backing from the lead Provider who oversees the overall program and provides full support to all team members and will be responsible for leading the nurse care managers and the clinical support team in delivering excellent cardiac care to heart failure patients . The NP will participate in the development of policy and procedures, reinforcement of clinical protocols and training of new hires. This role is also responsible to ensure that the RN care managers are covering schedules so that there are no gaps in coverage or licensure.

Responsibilities

  • Oversee initial intake and consultation of new patients
  • Execute the treatment plan as defined by key performance indicators and service line agreements while maintaining the patient as central to the process
  • Order self-monitoring devices such as weight scale, blood pressure monitor and oximetry systems.
  • Oversee clinical staff and care managers as they coordinate care with the Heart Failure Clinic.
  • Serve as a resource to answer any clinical questions from the team and provide ongoing collaborative education to both patients and the health care team members.
  • Assume co-management of the heart failure patients who are not achieving targets to reassess the treatment regimen, review barriers, and assess engagement of patients when needed.
  • Engage the lead Provider for complex cases when needed.
  • Manage and supervise the day to day activities of RNs and/or other care personnel, including hiring, firing and training of staff.
  • Review new or updated protocols and ensure standards of care are being maintained
  • Develop quality reports to present to direct VP of Care Management
  • Provide guidance to VP of Care Management on technology needs and optimizations
  • Ensure that QA reports are being completed regularly per protocol
  • Mentor RNs and/or other care personnel on managing workforce productivity, following all new protocols through implementation, proper training of staff, and follow up to ensure standard practices are being completed
  • Timely escalate clinical concerns, HR concerns, patient complaints, and safety concerns to appropriate leadership
  • Serve as an escalation point for challenges with vendors, device challenges, and patient needs
  • Meet regularly with RNs and/or other care personnel to ensure that KPI metrics are meeting minimum standards and support any action plan that needs to be implemented for short falls in KPIs
  • Timely report any technology challenges with details to IT and follow to resolution.
  • Inform VP of Care Management of any ongoing issues and plan for resolution
  • Work closely with Division Lead, NP Diabetes Care Management and other divisions as they develop
  • Assist with coverage in other programs as deemed necessary by Team Leads/ Project Managers for urgent staffing situations.

Qualifications/Requirements

  • 5+ years of cardiac or critical care experience as a Nurse Practitioner
  • 2+ years managing care teams
  • Experience and ability to function as a Care Manager while team members are out
  • Experience with hiring and executing disciplinary action plans
  • 5 years of triage experience
  • 2+ year of virtual care experience
  • Licensed in Texas, Connecticut, New Jersey
  • Multi-state licensed
  • Knowledge and experience with the four pillars of heart failure care.
  • Passion for improving patient outcomes long term.
  • Ability to work with a clinically advanced, high-risk population with comprehensive staff collaboration
  • Possibility to endorse to other states as needed
