JOB DESCRIPTION:
The remote licensed practical nurse (LPN) is responsible for assisting the onsite MA, LPN, RN in collaboration with the provider and other members of the health care team with all clinical services available at the clinic through the EMR system. Clinical responsibilities to include working the task list of needs for the patients through the EMR and communicating with the provider, care team and patient.
Job duties include but are not limited to:
- Communicating results and education to patients through phone or portal and document it within the EMR.
- Perform medication prior authorizations.
- Documents all patient/family interactions/responses, medications dispensed/prescribed, and test results in medical record.
- Assist the care team in working the nursing tasks through the EMR.
- Communicate closely with the nursing and provider team
- Ensures all patient information is in accurate locations of the patient chart for quality reporting and data extraction purposes.
- Coordinate and manage care of patients seen by provider including but not limited to following up and tracking testing, referrals.
- Stays informed of insurance requirements and guidelines for patient care. Performs prior authorizations and acts as patient advocate.
- Regularly monitors patients as appropriate to promote wellness and health maintenance including preventative and chronic care.
- Promotes wellness by providing patient education materials and communicating provider advice/instructions.
- Schedules, coordinates and communicates referrals to specialist and services ordered by the provider.

JOB QUALIFICATIONS:
- Experience:
- Minimum Required Experience: Ability to multi-task, good organization skills, effective communication skills.
- Preferred Experience: 12-year experience working in a clinic setting
- Education:
- Minimum Required Education: Completion of accredited Vocational/Technical Nursing Program
- Licenses:
- Minimum Required Licenses: Unencumbered active Kansas nursing license
