We are seeking an experienced MDS Coordinator to join our team. The ideal candidate will have a strong background in patient care and possess knowledge of MDS processes. An ideal candidate will have worked in skilled nursing and have previous MDS experience.

This position may work remotely. Travel to a Nye location may be required for initial training/orientation. Hours may be flexible.

Position Summary
Develops, plans, coordinates, organizes, directs, and evaluates the MDS and Care Planning activities/systems for the campus in accordance with scope of practice, doctors’ orders, established company policies and procedures, standards, and guidelines; current federal, state, and local laws and regulations governing the nursing department and campus; and as directed by the supervisor by performing the following duties personally or through subordinate team members.:

In this role you will:

  • Utilize Point Click Care software to manage patient data and documentation
  • Coordinate the due dates for residents MDS’, CAAs, and care plans and notify the care plan team
  • Coordinate the review and revision process of the resident MDS, CAAs, and Care Plans as required including 14- day admission, quarterly, annual, and significant change for non-medicare residents and 5, 14, 30, 60, 90-day, EOT/SOT/COT, and significant change for Medicare residents
  • Communicate with resident’s responsible party and staff regarding any changes, problems, concerns, and/or special needs residents may have
  • Assist to coordinate services to assist the resident to meet their needs which may include, but is not limited to PT, OT, ST, audiologist, psychiatrist, dentist, optometrist, and/or other referrals as indicated.
  • Coordinate discharge planning for patients
  • Work closely with managed care systems to optimize resident outcomes.

Qualified candidates will have:

  • Graduate from an accredited school of Nursing approved by the State of Nebraska.
  • Possess a valid RN license from the appropriate state. This shall include maintaining required CEU’s.
  • Previous experience in MDS coordination or a related role
  • Proficiency in Point Click Care software preferred
  • Familiarity with utilization management and discharge planning processes
  • Excellent communication and interpersonal skills.
