The Opportunity:
The Nurse Coach (NC) plays a key role overseeing the coordination and delivery of services for Sentrex Health Solution’s patient support programs. The NC serves as the main point of contact for their assigned healthcare providers (HCPs), ensuring patients have seamless access to the program and their medication. This role involves working collaboratively with the Care Coordinator, patients and caregivers, insurance companies, pharmacies, physicians, and other healthcare professionals to secure medication coverage, timely access, and manage patient cases.
A Day in the Life (What you will do here):
- Register HCPs and patients into the program, ensuring timely enrollment.
- Act as the main contact for HCPs and patients throughout treatment.
- Create a personalized infusion experience tailored to the unique needs of both the patient and their caregivers.
- Answer inquiries related to program services, disease state, and products.
- Provide education and answer questions related to T1D and the therapeutic journey.
- Follow case-management protocols, including monitoring, data collection, follow-up, and adverse event reporting.
- Assist physicians in preparing insurance documentation by reviewing patient history.
- Enter relevant data during program service delivery.
- Attend client meetings and conferences on disease and product information if required.
- Report adverse events to Health Canada and enter data into internal and third-party systems.
- Maintain confidentiality of patient and corporate information.
- Liaise with physicians, pharmacies, and service providers to ensure coordinated care and resolve issues.
- Provide proactive support and advocacy to patients and HCPs.
- Follow program protocols to ensure service continuity.
- Advocate for patient coverage decisions and follow up as needed.
- Meet service level requirements for case management, including phone response and timely adverse event reporting.
- Perform additional duties as assigned by the Manager.
- Adhere to company policies and procedures.
- Foster teamwork within internal support teams.
- Proven experience in delivering psycho-social support, helping individuals navigate personal challenges, emotional concerns and promoting mental well-being.
- Assist with other duties as assigned by the manager.
What you need to ensure you are set up for success:
- Bilingual in English and French (Required)
- Registered Nurse (RN) or Registered Practical Nurse (RPN) with current registration in a provincial College of Nurses in Canada
- 2-4 years of clinical experience in diabetes care
- Experience as a Diabetes Nurse Educator (current certification not required)
- Experience in Patient Support Programs is highly desirable
- Experience in delivering psycho-social support
- Experience working with pediatric patients
- Strong customer service orientation and focus
- Highly organized with attention to detail and critical thinking skills
- Strong business acumen and professional phone etiquette
- Capable of handling sensitive issues with discretion
- Passionate about patient advocacy
- Adaptable and able to efficiently manage multiple tasks
- Excellent problem-solving abilities with a focus on resolving issues quickly and effectively
- Exceptional verbal and written communication skills
- Must be able to work from home and have a quiet, private home office space
- Consistently meets deadlines and holds oneself accountable for results
- Comfortable working with various technologies, including CRM systems and smartphones
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, etc.)
