About Your Work
The Care management team is looking for a leader with Care Management experience who can lead a team of Health Navigators and Community Health Advocates supporting care management programs for all lines of business. The Supervisor, Care Management will report to the Manager, Outpatient Care Management. This will require a rigorous oversight of all regulatory aspects of care management.
Your focus will be on:
- Acts as a change agent in order to improve performance results, provides organizational effectiveness leadership to achieve department goals (people/process/technology).
- Responsible for a team of Care Managers, Care Coordinators, Health Navigators and Community Health Advocates (CHA) that focus on high quality patient care and other high member touch programs.
- Collaboratively works across multiple business areas to obtain agreement from all impacted parties when addressing cross functional organizational problems.
- Partner with Manager, Care Management to ensure team readiness for new program implementations with the goal of ensuring all members have access to the right care, at the right time, and at the right benefit at the right price on their effective date and all operational metrics are within agreed upon service levels.
- Maintain training and learning development plan for team. Ensures ongoing business process management using continuous process improvement practices, and implementation of technology to drive greater efficiency, productivity and customer experience.
- Responsible for key Care Management metrics related to compliance and contractual obligations by ensuring teams are within compliance in accordance to regulatory requirements.
- Maintains staffing and development of Care Managers, Care Coordinators, Health Navigators and CHA’s.
- Reviews results from key performance indicators and adjusts accordingly.
- This position will lead a team of 20 Care Managers, Care Coordinators, Health Navigators and Community Health Advocates.
The Skills, Experience & Education You Bring
- Current California unrestricted RN or LVN license required
- 2-3 years experience successfully managing people and leading teams.
- 5+ years’ combined experience managing programs, change initiatives, and/or CMS and DHCS audits and regulatory compliance.
- Managed care experience required
- Experience in care management, at a health plan or similar health care organization structure.
- Strong independent judgment, critical and analytical thinking, and problem-solving skills required.
- Experience with Power BI, Microsoft applications and other data analytics software
- Ability to multi-task, prioritize and deliver in a demanding and constantly changing corporate environment.
- Continuously learner; strong emotional intelligence.
- Ability to build trusting relationships
- Strong code of ethics and ability to work with internal and external stakeholders
- Proven record for driving to results through data-driven decision making
- Must demonstrate a sense of urgency and ownership to drive issues and tasks to completion.
- Ability to effectively manage and measure results to KPI’s.

The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. Physical Requirements:
Office Environment – roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.
