Job Summary

We are seeking an experienced Nurse Navigator to join our client’s team. This position is responsible for developing, assessing, and coordinating clinical, pharmacy, and care education activities to provide quality, cost-effective healthcare outcomes. The Nurse Care Navigator supports members and providers in identifying prescription drugs on the formulary that align best with the patient’s needs, ensuring optimal medication management, patient education, and care coordination.


Key Responsibilities

  • Develop, assess, and coordinate clinical and pharmacy care education activities based on member needs.
  • Support members and providers in identifying prescription drugs that align with patient needs.
  • Enhance patient care and outcomes through optimal medication management and patient education.
  • Collaborate effectively within a healthcare team to ensure comprehensive care.


3-5 Must Have Skills/Qualifications

  • Active RN License in Texas
  • Experience in clinical and managed care settings.
  • Strong understanding of various medications, their uses, and potential side effects. (Chronic conditions medications like diabetes, heart meds, COPD etc. No oncology)
  • Familiarity with health plan prior authorization and appeal processes.
  • 3+ years experience in similar role
  • Broad level experience across pharmacy, clinical, different specialties, strong UM experience, CM
