Job Title: LVN Care Navigator

Job Description

The LVN Care Navigator will be responsible delivering coordinated, person-centered care management for long-term care members by developing service plans, educating stakeholders, ensuring regulatory compliance, and collaborating across interdisciplinary teams to enhance health outcomes and service quality.

Responsibilities

  • Coordinate holistic care management by developing and assessing personalized service plans for long-term care members, ensuring quality and cost-effective healthcare outcomes.
  • Educate members and caregivers on available services, benefits, and procedures, while facilitating access to community resources and providers to meet medical and socioeconomic needs.
  • Monitor and document care plans and member outcomes, perform on-site assessments, and maintain compliance with state, federal, and payer regulations.
  • Collaborate with interdisciplinary teams and provide feedback to leadership to improve service delivery and care quality.

Essential Skills

  • Active LVN license
  • Strong communication skills.
  • Strong computer/documentation skills.

Work Environment

This is a full remote work-from-home position. The hours are Monday through Friday from 8:00am to 5:00pm. The dress code is business casual

Pay and Benefits

The pay range for this position is $19.00 – $21.00/hr.
