Job Description
S pectrum Healthcare Resources has an opportunity for a Registered Nurse Disease Managers (RNDM). These will be completely remote positions, working entirely from the Nurse’s home, however, Hawaii residency and timezone work is a requirement. The Nurse will be performing assessments, education and training to coordinate care for the dependents of our Nation’s Active Duty and eligible retirees.
Requirements
- Bachelors Nursing Degree
- Hawaii state license and its preferred to have an additional state nurse license, preferably compact state licensure, but not required.
- At least 2 years’ experience working as an RNDM, preferably in a remote or hybrid environment with Behavioral Health Experience strongly preferred.
- Preferred: Possess current certification in disease-management related area such as:
- Chronic Care Professional (CCP) by Health Sciences Institute
- Certified Health Coach (CHC) by National Society of Health Coaches
- Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators
- Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board
- Highly organized, self-directed worker able to function in a high-volume environment without distractions.
- Strong verbal and written communication skills.
- Proficient level of experience with Microsoft Office applications and strong technical aptitude.
- Must have access to secure, high speed internet.
Work Schedule
- Monday – Friday
- No nights and weekends
- 8 hours a day
Salary Range: $85k – $90K
