Job Description Summary
The Incidental Findings Navigator reports to the System Quality, Accreditation, and Education Director (Imaging Services). Under limited supervision, the Navigator owns the end-to-end coordination of timely, appropriate follow-up for incidental and/or concerning findings identified on radiology reports.
This role ensures closed-loop communication among patients, ordering providers, radiologists, primary care teams, and specialty services; manages tracking and documentation within the electronic health record (EHR) and follow-up registry; and escalates barriers to care to prevent patients from being lost to followup. The Navigator works collaboratively across the assigned division to support patients,
standardize processes, and drive measurable improvements in quality, safety, and experience.

Job Description
Primarily remote position.
Expected travel to CHS Division as needed.
Key Responsibilities Triage & Prioritization:
- Review radiology reports daily; categorize findings by risk/urgency using system guidelines; initiate follow-up pathways.
- Closed Loop Coordination: Notify patients and providers, facilitate orders and referrals, schedule follow-up imaging/consults, and verify completion.
- Tracking & Documentation: Maintain a real-time registry; document outreach, shared decision-making, and outcomes in the EHR; ensure data integrity.
- Escalation & Barrier Removal: Identify gaps (e.g., access, insurance, transportation); escalate per protocol to ensure timely care progression.
- Communication & Education: Serve as the point of contact for care teams; provide education on follow-up protocols, letters, and message templates.
- Quality & Compliance: Align workflows with system policies and accreditation standards; support audits and continuous improvement initiatives.
- Collaboration: Partner with radiologists, ordering providers, PCPs, navigation, scheduling, and population health teams to standardize division-wide practice.
- Reporting: Produce routine dashboards on volume, timeliness, closure rates, and lost – to follow-up; present trends and improvement actions.
Qualifications (Required):
Clinical background (RN, RT(R), APP, or related) and bachelor’s degree in a health discipline
Must be registered with the American Registry of Radiologic Technologist or South Carolina Board of Nursing as a Registered Nurse with 3 – 5 years in nursing, imaging modality care coordination or navigation.
The ability to view the hospital experience through the eyes of the patients and their family members in order to anticipate their needs and concerns. Strong working knowledge of hospital departments and how each affects patient throughput and satisfaction.
Fluent in Epic/EMR related applications.
- Proficiency with EHRs, registries, and reporting tools; strong written/verbal communication and customer-service skills. Demonstrated ability to manage complex caseloads, prioritize, and work independently with minimal supervision.
- Must maintain a secure, HIPAA-compliant, have remote workspace and be able to travel to CHS Division sites as required.
