Our nurses are part of a team of clinical and non-clinical Care Coordinators who have the ability to work within one of our Patient Engagement and Access Centers or remotely. The perfect nurse for us is one who is confident and capable of working independently and is an excellent communicator – which means you are a good listener and can communicate information clearly and effectively.
A day in the life of a nurse on SCP’s Care Coordination team is filled with responding to incoming and outgoing telephone calls, voice messages regarding clinical issues. You will help patients make health care decisions regarding the appropriate level of care, educating on their general health needs, answering questions about tests, treatments, procedures, and healthy lifestyle change.

Responsibilities:
- Ability to use critical thinking skills and respond quickly and professionally under stressful, sometimes emergent situations.
- Adheres to and participates in the Care Management process.
- Participates in staff conference regarding patient care.
- Provide telephonic clinical assessment and nursing triage to a wide variety of patients across the health continuum to culturally diverse population recently discharged from Emergency Room.
- Provide patient education to improve self-efficacy and help deliver better patient results by helping adhere to medication regimens, discharge instructions and help identify abnormal symptoms and which steps to take when issues arise.
- Assists in directing patients to the most appropriate level of care.
- Encourages patients to participate in other external programs or resources available to them in order to improve their health.
- Ability to answer questions from patients following ED and inpatient admissions.
- Ability to extract pertinent data from a multitude of patient electronic medical record systems in order adequately provide reliable information and education specific to the patient plan of care.
- Interact with patients using a pre-determined call flow before and after surgical intervention.
- Patient outbound follow up calls as necessary.
- Work closely with other members of the team to ensure adequate coordination.
- Must be flexible and have the ability to learn and retain multiple roles across care coordination services.
Knowledge, Skills, and Abilities:
- Must have excellent verbal and written communication skills with all members of the healthcare team.
- Must have excellent organizational skills and ability to complete competing priorities.
- Decisiveness and attention to detail as well as proficiency with technology, including computers, software applications, phone systems, etc.
- P roficient computer skills utilizing and navigating multiple applications at one time including Citrix, Outlook, MS office, VPN, and basic computer problem solving skills.
- Must have excellent time management skills in order to complete multiple tasks effectively and efficiently.
EDUCATION (Required):
- Graduate of US accredited nursing program with an active unrestricted RN license in good standing.
FIELD OF STUDY:
- Nursing- required
WORK EXPERIENCE/QUALIFICATIONS:
- Must be a RN with a minimum of 4 years in one of the following: ED, Med Surg, Home Health or Hospice.
- Excellent customer service skills including professional phone voice, empathy, and active listening skills.
- Excellent oral, written and interpersonal communication skills.
- CERTIFICATES AND LICENSES:
- Must possess current licensure as a Registered Nurse in the state of practice.
