Position Summary
The Clinical Coordinator is responsible for supporting the successful execution of Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and clinical implementation processes across partnered clinics. This role ensures operational readiness prior to go-live, maintains communication and scheduling accuracy, reviews clinical documents for quality and compliance, manages protocol updates, and assists with ongoing audits and AI-supported workflows. The Clinical Coordinator plays an essential role in ensuring clinics, patients, and internal teams have a smooth, well-organized, and clinically sound onboarding experience.
Key Responsibilities
1. Go-Live Coordination & Scheduling
Conduct all go-live presentations with clinics.
Send reminder emails to clinics one day prior to scheduled go-live meetings.
Manage all reschedules, cancellations, and calendar adjustments promptly.
Maintain accurate documentation of scheduling changes and clinic communications.
2. Clinical Review & Quality Assurance
Review clinical protocols and treatment plans in Monday.com after they are signed by the clinic prior to go-live.
Identify any missing information, inconsistencies, or protocol discrepancies.
Communicate required updates, needed corrections, or clarification requests to the appropriate teams.
Ensure all clinical documentation aligns with operational standards prior to activation.
Manage protocol and parameter updates received from Support and Customer Success, ensuring all changes are accurately applied, communicated, and reflected in clinical workflows.
3. AI Intake & Workflow Support
Monitor and work AI-generated RPM intake and flag queues in between assigned calls.
Provide structured feedback on AI performance, accuracy, and opportunities for improvement.
Participate in AI testing, pilot evaluations, or workflow adjustments as assigned.
4. Chart Review & Billing Support
Assist with chart reviews to help meet billable unit goals.
Perform documentation validation and ensure clinical entries meet compliance expectations.
Support the clinical audit process as needed, including follow-ups and corrective actions.
5. Cross-Functional Communication
Serve as a liaison between clinics, clinical operations, implementation, and technical support teams.
Communicate updates, issues, and action items clearly and promptly to ensure continuity of care and operational success.
Escalate concerns when clinic readiness or documentation accuracy may impact go-live timelines.
Qualifications
Must obtain and maintain a multistate LPN/LVN license.
Experience in clinical operations, healthcare coordination, RPM/CCM programs, or similar preferred.
Strong communication and organizational skills.
Ability to work independently in a remote environment.
Comfort with technology platforms such as Monday.com, EHRs, and AI-enabled workflows.
Detail-oriented with strong follow-through skills.
Ability to multitask and prioritize across multiple clinics and deadlines.

This is a fully remote role requiring reliable internet access, a distraction-free work environment, and availability during assigned clinic hours. Occasional meetings outside typical hours may be required based on clinic scheduling or operational needs.
