About the Role:
- Education and Counseling: This role is a remote, patient-facing role. As part of our RN team, you will provide education about a patient’s diagnosis, test results and treatment plan.
- Collaboration: Collaborate closely with cardiologists, PCPs and other healthcare professionals to ensure comprehensive and coordinated care for patients. Discuss complex cases and participate in multidisciplinary care teams. Utilize clinical decision making to escalate concerns to the care team
