As an integral member of the CHAT or ACO team, the Case Manager RN evaluates and manages the changing health and service needs of the patient.  The Case Manager RN is responsible for providing optimum individual care for the patient/family unit, using the nursing process and interdisciplinary team members.  Collaborates with the patient’s Primary Care Physician and other health team members to create and implement a written care plan within the time frame identified in documentation standards.

Required Qualifications:    

  • Upon hire: Washington Registered Nurse License

Preferred Qualifications:

  • Bachelor’s Degree Nursing
  • 3 years Current experience in a health care environment with home health case management experience.

Posted are the minimum and the maximum wage rates on the wage range for this position.  The successful candidate’s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
