As an integral member of the CHAT or ACO team, the Case Manager RN evaluates and manages the changing health and service needs of the patient. The Case Manager RN is responsible for providing optimum individual care for the patient/family unit, using the nursing process and interdisciplinary team members. Collaborates with the patient’s Primary Care Physician and other health team members to create and implement a written care plan within the time frame identified in documentation standards.

Required Qualifications:
- Upon hire: Washington Registered Nurse License
Preferred Qualifications:
- Bachelor’s Degree Nursing
- 3 years Current experience in a health care environment with home health case management experience.
