License must be clear of any clinical and/or board practice restrictions.</li><li><strong>NY State only:</strong> Minimum of 3600 hours of practice experience, or with 3 years of clinical experience (autonomous provider), in a primary care or longitudinal care setting</li><li>Board Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant</li><li>Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements.</li><li>Active Drug Enforcement Administration (DEA) license.</li><li>Current Basic Life Support Certification for health care providers.</li><li>Proficiency in electronic medical record (EMR) systems.</li><li>Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.</li><li>Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant</li><li>Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements. Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
