What you’ll be doing:
The Triage Nurse – RN is the clinical frontline of the Value-Based Operations team. This position is responsible for answering blended inbound and outbound telephone calls while working in conjunction with the administrative staff and external programs to manage patient care.
The Triage Nurse-RN responds to incoming telephone calls, voice messages and/or text messages regarding clinical issues. The Triage Nurse-RN helps patients make health care decisions regarding the appropriate level of care, managing their general health needs, answering questions about tests, treatments, procedures, and healthy lifestyle changes. The Triage Nurse-RN will be responsible for utilizing clinical knowledge and the nursing process to quickly identify if one of our standardized clinical protocols is needed in order to safely resolve the needs of the patient.
Responsibilities:
- Use critical thinking skills and respond quickly and professionally under stressful, sometimes emergent situations
- Provide clinical assessment and nursing triage to a wide variety of patients across the health continuum and/or health education to culturally diverse populations
- Assists in directing patients to the most appropriate level of care
- Provide telephonic health information and nursing education
- Assists and empowers patients to make better health care decisions
- Helps patients to participate in other external programs or resources available to them to improve their health
- Work in collaboration with Telehealth providers by referring appropriate patients who may benefit from a “virtual care visit”
- Provide outbound follow-up calls as necessary
Qualifications:
- You have an active, unrestricted RN licensure with at least five years of RN clinical nursing experience required
- You are able to work at least 10 hours per month; hours vary depending on the shift selected
- You are able to cover a full 4 to 12-hour shift to meet the required monthly hours
Education:
- Associates or Bachelors of Nursing degree (ADN or BSN) required
Certifications and Licenses:
- Active & unrestricted RN license (compact or able to obtain a compact license)
Experience:
- Previous Emergency Department, ICU, EMT, or Triage experience (inpatient patient bedside assessment considered if in an acute patient setting), required
Knowledge, Skills, and Abilities:
- You are a team player with excellent verbal and written communication skills with all members of the healthcare team (nurses, physicians, care managers, social workers, etc.)
- You have excellent organizational skills and can prioritize and complete tasks when there are multiple competing priorities
- You are decisive and have attention to detail as well as proficiency with technology, including computers, software applications, phone systems, etc.
- You have reliable internet service -required
- You are able to complete training related to new programs within a maximal time frame of 3 weeks from the date of hire required.
- You have knowledge of Hospital Electronic Medical Records (EMR) systems
- You are able to manage patient information according to HIPPA guidelines
- You have the ability to work under stressful conditions or irregular hours

- 24/7 coverage- shifts cover 7 am-7 pm and 7 pm-7 am (must work a total of 10 hours per 4 week shift to maintain PRN status)
- May require occasional night/evening and weekend or holiday work
Pay Range:
27.00 – 40.00 USD per hour
This range represents the anticipated base salary for this role. Actual compensation will be determined based on experience, qualifications, and internal equity considerations.
