PART TIME-DAYS-REMOTE
Scope of Work
Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
Qualifications
- Bachelor’s degree or equivalent Required.
- Master’s degree or equivalent Preferred
- 2 years of relevant experience required
- LIC-Registered Nurse (RN) – STATE_MI State of Michigan Excludes CV Administration, HM Patient Care Logistics, Health Comm – Department, GM-Community Education, Lifestyle Medicine, PD HDVCH Child Protection, PH Medical Affairs Admin, PH Pre-Service Utility Management Upon Hire Required Or
- LIC-License Practical Nursing (LPN) – STATE_MI State of Michigan Upon Hire Required Or
- CRT-Registered Dietitian (RD) – CDR Commission on Dietetic Registration Upon Hire Required Or
- LIC-Master Social Worker (MSW-Master) – STATE_MI State of Michigan Upon Hire Required Or
- LIC-Physical Therapist – STATE_MI State of Michigan Upon Hire Required Or
- CRT-Occupational Therapist, registered (OTR) – NB-COT National Board for Certification in Occupational Therapy Upon Hire Required Or
- CRT-Speech Language Pathologist – ASHA American Speech-Language-Hearing Association Upon Hire Required Or
- CRT-Nuclear Medicine Technologist – NMTCB Nuclear Medicine Technology Certification Board Cardiovascular Dept. Upon Hire Required Only
- CRT-Basic Life Support (BLS) – AHA American Heart Association Excludes Trauma Dept. and Community Blue Envelope Upon Hire Required
- CRT-Basic Life Support (BLS) – ARC American Red Cross Excludes Trauma Dept. and Community Blue Envelope Upon Hire Required
