We are seeking a dedicated and compassionate Registered Nurse Case Manager, with pediatric experience, to join our team at Footprints Case Management. In this pivotal role, you will be responsible for coordinating patient care, ensuring effective utilization of healthcare resources, and advocating for patients throughout their treatment journey. Responsibilities
- Assess and evaluate patient needs to develop individualized care plans.
- Coordinate with multidisciplinary teams to ensure comprehensive patient care.
- Monitor patient progress and adjust care plans as necessary.
- Facilitate communication between patients, families, and healthcare providers.
- Conduct utilization management reviews to optimize resource allocation.
- Educate patients and families on treatment options and care processes.
- Maintain accurate documentation of patient interactions and care plans.
- Stay updated on best practices in case management and healthcare regulations.
Qualifications
- Current Registered Nurse (RN) license in the state of North Carolina.
- Experience in managed care or case management preferred.
- Strong organizational skills with the ability to manage multiple cases effectively.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Ability to work collaboratively within a team-oriented environment.
