About The Role
As a Registered Nurse, you’ll provide day-to-day collaborative support to our internal Medical and Psychiatric providers to ensure smooth and effective patient care. You’ll coordinate care across internal and external teams, monitor critical lab results and vitals, and triage patient requests. Additionally, you’ll provide condition-specific education to patients and families, complete accommodation paperwork, and perform solo visits when necessary.
Responsibilities
- Act as a liaison for your own patient panel of medically stable patients.
- Monitor and triage lab orders/results, inform patients and families of provider’s instructions, and coordinate care between internal and external providers.
- Complete accommodations paperwork for FMLA, short-term disability, home hospital, etc.
- Manage the RN phone line to facilitate care coordination with external providers.
- Follow medication triage protocols by transmitting and transcribing medication orders, refills, fielding questions from families/patients, etc.
- Provide families with condition-specific education and medical management support.
- Triage emergent issues in a time-sensitive manner and in alignment with our triage protocols.
- Perform other duties as assigned.
Qualifications
- Current and unrestricted license as a Registered Nurse in at least one state in the USA, compact license preferred.
- 2+ years of experience as a Registered Nurse in a clinic, homecare, ambulatory, hospital or inpatient setting
- Experience coordinating care with multiple providers as well as patient support systems.
- Ability to manage multiple requests and prioritize accordingly.
- Excellent communicator with a high level of adherence to confidentiality.
- Successful experience working on a multi-disciplinary care team and a collaborative mindset.
- Ability to work well autonomously within the outlined procedures and clinical protocols.

Physical Demands
Work is performed 100% from home with no requirement to travel. This is a stationary position that requires the ability to operate standard office equipment and keyboards as well as to talk or hear by telephone. Sit or stand as needed.
