Job Summary:
The Cardiac Device Registered Nurse (RN) will work under the direction of the Medical Director of the Heart Failure Remote Monitoring Program and the Heart Failure Program leadership. In this role, the RN will be accountable for the effective management and coordination of care for patients utilizing remote cardiac monitoring technologies. These include, but are not limited to: hemodynamic monitoring systems (CardioMEMS, Endotronix, HFMS), cardiac implantable electronic devices (CIEDs), pacemakers, implantable cardioverter defibrillators (ICDs), implantable loop recorders, event/holter monitors and wearable cardiac defibrillators. Serves as a liaison across the Heart and Vascular Institute (HVI) Service Line, fostering collaboration and continuity of care for the cardiac device patient population. Responsibilities will include data collection, care coordination for both patients and team members, education and patient navigation – with a particular focus on seamless transitions from inpatient to outpatient settings. In partnership with a multidisciplinary team, the RN will support efforts to improve quality outcomes and deliver patient-centered care for individuals and families served by the Heart Failure Remote Monitoring Program.
- Bachelor’s Degree – Nursing
- Registered Nurse
- Basic Life Support (BLS)
- Minimum one (1) year of Cardiology Nursing experience or two (2) years of general nursing experience with a strong interest in Cardiovascular care.
