Are you a compassionate RN with a passion for guiding patients through complex care journeys? We’re looking for a RN Care Manager to join our Care & Case Management team at a Health Plan.
In this telephonic, remote role, you’ll engage with high-risk and rising-risk members—particularly around discharge planning and transitions of care—ensuring holistic, patient-centered support. You’ll collaborate with a multidisciplinary clinical team to deliver innovative, integrated care that changes lives.
🔍 What You’ll Do
âś” Proactively engage members post-discharge
âś” Conduct virtual assessments covering medical, cognitive, and social health
âś” Create holistic, individualized care plans
âś” Coordinate clinical and social resources
âś” Partner with hospitals and internal teams to ensure smooth transitions
🎯 What We’re Looking For
âś” BSN required
âś” RN license (CA and/or Compact preferred)
âś” 5+ years clinical nursing experience
âś” 2+ years in case or care management (TPA/health plan preferred)
âś” Strong remote work skills and tech-savvy
âś” High empathy, adaptability, and clear communication
