Summary
The Population Health Team is the engine driving this change. We don’t just manage cases—we leverage deep data insights and clinical
expertise to proactively steer our most complex members toward their optimal health. As a Clinical Care Navigator RN, you are the clinical
anchor for members with high-acuity needs, serving as the bridge between data-driven insights and compassionate, human-centered care.
Essential Functions: What You’ll Do
- Clinical Strategy & Navigation: Act as the primary clinical point of contact for members with multi-morbidities, chronic conditions, and complex psychosocial needs. You aren’t just following a script; you are architecting a personalized path to wellness.
- Data-Driven Advocacy: Utilize our data-first mindset and the critical Baseline Visit to identify gaps in care and opportunities for intervention before a crisis occurs.
- Holistic Assessment: Conduct clinical assessments that go beyond the surface, evaluating a member’s history, functional abilities, and health literacy to build a truly individualized care plan.
- Collaborative Leadership: Partner closely with non-licensed Care Navigators to provide clinical oversight, ensuring that educational outreach is medically sound and highly effective. You are aligned with a Care Navigator Pod to provide clinical expertise and intervention when needed.
- Transitions of Care: Navigate the complex continuum of care, from acute discharge planning to community resource integration, ensuring transitions are seamless and safe.
- Program Innovation: As part of a growth-stage team, you will help build the plane while flying it, contributing to the development and implementation of new clinical programs and workflows.
- Regulatory Excellence: Maintain the highest standards of HIPAA compliance, ethics, and clinical integrity while meeting key performance metrics that drive member outcomes.
Qualifications: Who You Are
The Essentials
- Education: Graduate of an accredited nursing program with an active, unencumbered RN license.
- Experience: Minimum of 2 years in Case Management within a health plan/managed care setting. You know the payer side of the house and how to navigate it.
- Clinical Acumen: Experience working with vulnerable populations and managing complex, high-acuity cases (e.g., chronic disease management, behavioral health integration).
- Tech-Savviness: You are comfortable in a Google Suite environment and can quickly learn new technology platforms.
Preferred
- Certifications: CCM (Certified Case Manager) or CMCN (Certified Managed Care Nurse).
- Education: BSN preferred.
- Bilingual Proficiency: Fluent in Spanish and English (verbal and written) is preferred.
- The Startup Mindset: You thrive in nimble environments. You are comfortable with ambiguity and enjoy the challenge of a fast-paced, changing landscape.
The Remote Setup & Expectations
- Reliability: You must have a dedicated, quiet, and secure home office space to maintain HIPAA compliance.
- Connectivity: High-speed, password-protected internet is a must (a stipend is provided).
- Schedule: Full-time (M-F, 8:00 AM – 5:00 PM local time). Flexibility is key as we support members across multiple time zones, often centered on Central and East Coast Times.
- Onboarding: To ensure a successful launch, we request no time-off requests within the first 120 days of employment (emergencies excepted).
Physical Requirements
- This is a sedentary role requiring extensive computer use, including vision abilities for close-up digital work and frequent communication via phone and video conferencing.
Travel
No travel is expected with this role, but may be optional for department offsites.
