Job Summary

The Clinical Intake Coordinator is responsible for managing the intake process for new referrals, ensuring all necessary information is accurately reviewed and processed. This role involves close collaboration with the Intake Team, Hospital Liaisons, Clinical Case Manager and other healthcare professionals to facilitate smooth admissions and transitions of care.

Key Responsibilities:

The Clinical Intake Coordinator’s key responsibilities include

  1. Referral Management
  2. Ability to process multiple referral requests from hospital systems
  3. Process referrals by coding, reviewing medication, insurance verifications, equipment needs and more
  4. Assess all referrals for clinical appropriateness and send referrals to clinical specialty programs like dialysis 

 

  1. Communication and Coordination:

·       Coordinate Referrals with the lead TCC and work collaboratively to determine answers.

·       Communicate concerns or denial requests to the Chief Nursing Officer

·       Ensure timely acceptance or denial of referrals within hospital referral platforms.

  1. Tasks: 
  2. Ability to identify Medicare, Medicaid and Commercial payors and request insurance authorizations when necessary
  3. Familiarity with Managed Care Contracts and reimbursement 
  4. Run background checks in accordance with Accolade Healthcare Policy 
  5. Ensure all information demographics, clinical review, insurance authorizations and DME is sent to the admitting facility 

Qualifications:

  • Must hold a State of Illinois Registered Nurse License
  • Proven experience in a clinical intake or similar role is preferred
  • Prior work experience in post acute care preferred 
  • Strong organizational and communication skills.
  • Ability to work collaboratively with a multidisciplinary team.
  • Proficiency in using hospital referral platforms and electronic medical records
  • Attention to detail and accuracy in documentation

  • Excellent interpersonal and communication skills.
  • Strong analytical and problem-solving abilities.
  • Proficiency in medical coding and understanding of medications and equipment needs.
  • Ability to manage multiple tasks and prioritize effectively.
  • Knowledge of Skilled Nursing Regulations and Capabilities as it related to resident care and admission policies
