Schedule Details/Additional Information:

Mon-Fri 7:30a-4:30p
Administrator on-call rotation for nights and weekends approximately 5x/year and may include holiday(s), this is a hybrid position with 90% remote work.

  • Prefer an experienced hospice clinician (at least 3-5 yrs of direct pt care and a strong foundation in hospice care)
  • Regulatory and Compliance Expertise – deep understanding of CMS, Medicare Conditions of Participation, and accrediting body requirements
  • Documentation and audit proficiency-ability to audit clinical records and identify gaps, inconsistencies, or compliance risks
  • Analytical and reporting skills – able to interpret data, identify trends, and communicate findings to leadership
  • Strong communicator (written and verbal) and educator – able to guide, correct and educate staff with diplomacy
  • Time management and independence – able to prioritize tasks, meet deadlines, and work independently w minimal supervision but is very team oriented
  • Tech proficiency – competent w EMR and data reporting

Major Responsibilities:

  • Assists leader in achieving compliance of EHR content with regulatory requirements, including accuracy of document and compliance with content of initial and comprehensive assessment and plan of care. Ensures accuracy of quality reporting, claims processing, assessment and plan of care processing, and audit preparation and review.
  • Creates and analyzes reports related to clinical EHR accuracy and compliance. Identifies improvement opportunities and assists in rectifying and communicating correction needs. Tracks necessary corrections and identifies and carries out educational needs; collaborates with education department to develop training based on department or agency trends.
  • Ensures clinical team documentation is in full compliance with all applicable accreditation body rules and regulations and agency policy and procedures. Ensures workflow processes are timely and cost efficient.
  • Performs and oversees chart audits and tabulates and analyzes findings. Assists in the development and implementation of performance and process improvement plans, on both an individual and department level, based on findings. Reports results to leader on a routine basis.
  • Monitors results of chart audits and assures that remedial action is taken. Identifies trends in audits and communicates results to leadership and the education department.
  • Audits clinical outcomes and service quality goals. Identifies, develops and participates in performance improvement plans that will enhance the quality of the services provided.
  • Identifies need for correction of charting deficiencies; assists in implementation of timely correction of charting deficiencies that impact the compliance of the EHR and the clinical workflow for the revenue cycle agency-wide.
  • Processes items on work queues, performs claims reviews, and ensures medication information is entered onto the claim accurately. Audits quality of work done by support staff related to revenue cycle practices.
  • Monitors and assures reliability of clinical assessments; tracks and remedies variances in outcomes and adverse events.
  • Develops collaborative and cooperative relationships with internal and external partners related to areas of accountability. Participates in department, agency, and system workgroups as indicated. Assists in establishing and maintaining policies and procedures that are consistent with the mission and strategic goals of Aurora at Home and Aurora Health Care.

Licensure, Registration, and/or Certification Required:

  • Registered Nurse license issued by the state in which the team member practices.

Education Required:

  • Bachelor’s Degree in Nursing.

Experience Required:

  • Typically requires 3 years of experience in clinical nursing.

Knowledge, Skills & Abilities Required:

  • Driver’s license issued by the state of Wisconsin, or Driver’s license issued by the state of Illinois.
  • Must have a thorough understanding of home health and hospice regulations, reimbursement and care, with familiarity of EPIC hospice and home health workflows.
  • Strong analytical, verbal and written communication skills.
  • Demonstrates an ability to organize, plan and delegate work effectively and efficiently.
  • Demonstrates an ability to solve problems and make independent judgments and decisions based on experience and good judgment.

Physical Requirements and Working Conditions:

  • Generally exposed to a normal office environment.
  • Exposed to care environment (facility or home setting).
  • Exposure to hazardous materials or potentially infectious body fluid is possible.
  • Position requires travel so may be exposed to weather and road conditions.
  • Operates all equipment necessary to perform the duties of the job.

Pay Range

$37.50 – $56.25
