What We’re Looking For


Trillium Health Resources has a career opening for a Complex Care Nurse to join our Transition to Community Living (TCLI) team. The TCLI team works in collaboration with the NC Department of Health & Human Services and other MCOs across the state to meet the goals specified in the Department of Justice (DOJ) Settlement Agreement. In the Integrated Care Nurse role, you’ll use your skills to provide input for Complex Care Coordination targeting members with chronic, unresolved or complex physical, behavioral health and social determinant needs. As a member of the TCLI team and in the Integrated Care Nurse role, you’ll be working to help members live their best lives.


On a typical day, you might:


Act as the liaison for Personal Care Referrals.

Discuss ED/hospitalization encounters with members to improve outcomes.

Utilize Evidence Based Practices to provide disease management education.

Effectively communicate treatment updates and other member related activities both verbally and in writing to appropriate personnel and agencies.

Link to needed psychological, behavioral, educational, physical evaluations, occupation therapy assessments and other specialist(s) as needed

Qualifications:


Education/Experience:

Fully licensed by the North Carolina State Board of Nursing as an RN required with a minimum of three (3) years as a Registered Nurse.


Preferred Experience:

Experience working with BH/MH/SU/IDD population


Knowledge of QM, UM procedures as well as experience in using data analytics for population health management preferred

License/Certification:

Must have a valid driver’s license

Prefer a Certified Managed Care Nurse Certification or certification in care management.


Location:

Remote within North Carolina.

Must reside in North Carolina to be considered for remote status.
