Position Summary

  • The Department of Care Management (CM) utilizes the Discharge Navigator role to enhance the operational and clinical effectiveness of the organization and department and to ensure seamless management of the day to day activities of staff engaged in discharge planning, care coordination activities, social work and capacity management.

Minimum Requirements

  • Bachelor of Social Work degree from an accredited school of social work OR an Associate Degree in nursing from an accredited college is required. 
  • Case Management or discharge planning experience is preferred.

  • Department/Location: ECU Health Medical Center (Greenville, NC) – Case Management Services
  • This position is full-time (40hrs/week) 8:30am – 5:00pm – hybrid remote 
  • No nights, call or weekends
