Overview: The Follow-up Nurse, RN is a highly specialized nurse that ensures patients or designees are informed of diagnostic tests and results after discharge from the Emergency Department. Communicates with internal and community providers, field calls from patients who have questions or concerns after leaving the hospital. Schedules patients for specialty treatments that are recommended after discharge. Provides follow-up to patients that require additional treatment. Works collaboratively with local health departments regarding reporting of communicable diseases. Stays up-to-date on follow-up best practices and communicable disease reporting requirements.
