What You Will Do
- Demonstrates clinical expertise with regards to age specific populations, including neonates, pediatrics, adolescents, adults and geriatrics.
- Provides complete accurate documentation of each phone call and follow-up.
- Communicate clear, concise information both verbally and in writing to all team members.
- Assists interdisciplinary group in providing 24 hour, 7 days per week care.
- Demonstrates initiative in identifying opportunities for self-development and enhancement of professional competency.
- Understands, applies and supports Hospice, Alliance, and Methodist LeBonheur Healthcare policies, procedures and standards.
Education/Formal Training Requirements
- Technical Degree or Diploma Nursing – LPN
Work Experience Requirements
- 1-3 years Home care/hospice
Licenses And Certifications Requirements
- Licensed Practical Nurse – Arkansas – Arkansas State Board of Nursing
- Licensed Practical Nurse – Mississippi – Mississippi Board of Nursing
- Licensed Practical Nurse – Tennessee – Tennessee Board of Nursing
Knowledge, Skills And Abilities
- Work is performed off-site; must provide own transportation to deliver paperwork and attend meetings.
- Ability to understand complex written materials, such as patient records.
- Ability to communicate verbally with all levels of associates, management, physicians, and patients and their families.
- Ability to work without close supervision, and to exercise independent judgement in a professional area.
- Ability to work within an interdisciplinary environment as a member of a team dedicated to shared patient/family responsibilities.
- Ability to organize multiple tasks and projects and maintain control of work flow.
