What You’ll Do
As a Patient Care Coordinator, you’ll play a vital role in our patients’ health journeys by:
- Conducting monthly telephonic outreach and assessing patient needs.
- Creating individualized care plans focused on physical, mental, and preventative health.
- Documenting visits in technology platforms and electronic health records (EHRs).
- Coaching patients through wellness, nutrition, and goal setting.
- Helping patients prepare for medical appointments and connecting them with resources.
What We’re Looking For
- Active Compact LPN license.
- Minimum 2 years of clinical experience (care coordination preferred).
- Strong tech confidence—comfortable with EMRs, Microsoft Office, and digital tools.
- Excellent communication and problem-solving skills.
Schedule & Compensation
- Full-time, 40 hours/week | Monday–Friday between 8 AM – 6 PM (patient’s time zone).
- Orientation & Training (first 2 months): $20/hr.
- Monthly bonus potential: up to $525.
- Referral bonuses: up to $1,000.
What To Expect After You Apply
- Timeline: The hiring process typically takes 2–3 weeks.
- Assessment: Qualified applicants complete a timed assessment covering computer literacy, basic math, and clinical knowledge.
- Important: You must use a desktop or laptop. Tablets and phones will not work.
- Interviews: Successful candidates will move forward to virtual interviews with our Recruiting Manager and Clinical Director.
