Remote LVN Care Navigator

Location: Remote but must be located in Corpus Christi, TX area

Schedule: Monday–Friday, 8:00 AM – 5:00 PM (no overtime required)

About The Role

  • We are seeking a Licensed Vocational Nurse (LVN) to join our team. The LVN will play a key role in member coordination, ensuring timely follow-ups, accurate documentation, and compassionate care delivery.

Key Responsibilities

  • Conduct follow-up calls with members, physician offices, DME agencies, and provider agencies.
  • Complete assigned tasks within expected turnaround times.
  • Track productivity and submit weekly reports.
  • Fill out required forms and document assessments accurately and on time.
  • Maintain HIPAA compliance while working independently from a home office.
  • Utilize computer systems and resources effectively to support care coordination.

Qualifications

  • Active LVN certification with the Board of Nursing (BON).
  • Experience in home health, managed care, or physician office settings.
  • Strong computer skills and reliable Wi-Fi.
  • Excellent verbal and written communication skills.
  • Proficiency with Microsoft Office (Excel, Word, etc.).
  • Quick learner with strong organizational skills.

Job Type & Location

This is a Contract position based out of Corpus Christi, TX.

Pay And Benefits

The pay range for this position is $20.00 – $25.00/hr.
