Job Summary:

The Home Health Intake Specialist is responsible for receiving referrals via both external portals and internal platforms, verifying eligibility and benefits and communicating this information to the business development and clinical operations teams quickly and accurately. The intake specialist must be able to multi-task and prioritize a large workload to meet timeliness benchmarks to maximize referral conversion. The Home Health Intake Specialist plays a critical role in managing new patient referrals and coordinating their entry into home health and hospice services.

The ideal candidate for this position has 1+ year of home health and/or hospice intake experience and is knowledgeable with navigating HomeCareHomeBase and Forcura.

Qualifications

Qualifications:

  • Licensed Vocational Nurse, licensed per the state requirements
  • 1 year of clinical intake experience is needed
  • 1 year of Home Health & Hospice experience is needed
  • HomeCare HomeBase experience is needed.
  • Forcura experience is needed.
  • Experience with insurance verification & eligibility preferred

Essential Functions:

  • ¨ Receive and appropriately document new referrals in a timely manner.
  • ¨ Provide effective communication to patients, staff members, other health care professionals and referral sources.
  • ¨ Respond to agency and patient needs in a professional and creative manner.
  • ¨ Demonstrate commitment, professional growth and competency.
  • ¨ Promote Agency philosophy and administrative policies to ensure quality of care.
