Member Care Coordinator


Location: Chicago, IL


Job Type: Full-time Contract, Remote (Training will be a few days onsite)


Pay: $25-29 per hour based on Benefits


Description: We are seeking a dedicated and compassionate Member Care Coordinator for a telephonic-based role. This position is responsible for conducting home health assessments and contacting identified members to inform and educate them on health care programs to address their personal health plan needs.


Essential Functions:

  • Conduct home health assessments and system updates
  • Perform outreach and follow-ups with members on their care plans
  • Build relationships to ensure care plan compliance
  • Educate members and update records
  • Check in with members on care plan goals
  • Meet production targets based on department needs
  • Support clinical team with non-clinical tasks
  • Handle member and provider inquiries
  • Report system issues to help desk
  • Update customer/provider data
  • Gather missing information via calls or correspondence
  • Liaise on claims and refer cases to clinicians
  • Coordinate with internal departments on concerns
  • Communicate professionally with all co-workers, management, and customers
  • Follow HIPAA, diversity, and compliance policies
  • Maintain confidentiality
  • Report updates to management and assist with special projects


Job Requirements:

  • Bachelor of Social Work or Psychology OR LVN/LPN with 1 year experience in managed care systems OR RN with 3 years of care coordination for a state managed or waiver program OR 3 years of managed care systems experience
  • Knowledge of medical terminology
  • Experience supporting member medical needs
  • Proficiency in Microsoft Office
  • Strong customer service skills
  • Excellent verbal and written communication
