Work Schedule: Monday-Friday, 8am-5pm
Location: work from home with some patient visits and weekly team meetings onsite (High Point office).
This position demonstrates competency in delivery of quality patient care to patients. The RN Case Manager is responsible for providing services to enhance continuity and coordination of care for patients enrolled in Care Connection in compliance with the Nurse Practice Act, organizational policy and procedures, and standards of care.
What are the essential duties day to day?
- Daily phone calls to check in on patients
- Plan, provide, supervise and document safe, competent and compassionate hospice nursing care for patients in accordance with physician orders and establish policies and procedures
- Completes nursing assessments, via telehealth or in person according to Plan of Care
- Completes admission/intake visits when assigned
- Offering guidance to patients when medical issues arise
- Develops and coordinates Plan of Care in collaboration with patient, significant others, and interdisciplinary team members to focus on needs of patient in order to assure satisfactory outcomes and achieve patient care goals
- Accurate and timely documentation and communication
- Provides direct patient care according to the Plan of Care
