Role Summary
We are seeking a Licensed Practical Nurse (LPN) to join our team as a Pediatric Care Manager. This role involves a mix of care coordination, prior authorizations, provider and patient communication, and medication management. You will collaborate closely with pharmacists, physicians, and caregivers to ensure seamless care for pediatric patients with complex medical needs.
Key Responsibilities
Prior Authorizations & Appeals
- Submit and track prior authorizations for medications, DME, and other treatments.
- Manage appeals for denied requests, coordinating with providers and payers.
- Maintain accurate records in Salesforce.
Patient & Caregiver Communication
- Under the guidance of a PharmD, serve as the primary point of contact for caregivers regarding medication management, prior authorizations, and care coordination.
- Provide education on medication changes, proper administration techniques, and potential side effects as directed by the pharmacist.
- Assist caregivers with vacation medication overrides and navigating out-of-state medication access to ensure continuity of care, in collaboration with the pharmacist.
- Ensure all PharmD recommendations are communicated to the caregivers and providers of the child’s care team.
- Conduct periodic check-ins with patients and caregivers to ensure treatment plans are followed and modify them with PharmD oversight.
Provider Communication & Coordination
- Obtain and update provider contact information to ensure efficient collaboration.
- Communicate recommendations and approvals via fax, email, phone, and portal messaging.
- Schedule and coordinate combined appointments with multiple providers when needed.
- Ensure all PharmD recommendations are conveyed to the child’s healthcare team, ensuring alignment in care decisions.
Medication & Pharmacy Coordination
- Manage medication acquisition, including refill requests, new prescriptions, and out-of-stock medications.
- Facilitate pharmacy transfers and ensure patients receive their medications in a timely manner.
- Assist in ordering PGX (pharmacogenomic) and Tempus testing when appropriate.
Care Coordination & Referrals
- Work with care managers, providers, and caregivers to ensure a holistic care approach.
- Coordinate with skilled nursing, home health aides, and DME companies.
- Help facilitate referrals to specialists or additional healthcare services.
Administrative & Communication Support
- Answer and direct calls, voicemails, and messages to the appropriate team members.
- Maintain up-to-date documentation in Salesforce.
- Provide timely status updates on submitted authorizations and recommendations.
- Other duties as assigned.
Qualifications
- LPN Required; active license in one or more U.S. states.
- Minimum of 3 years experience in pediatrics, case management, prior authorizations, or care coordination preferred.
- Strong communication skills with the ability to work directly with providers, caregivers, and pharmacists.
- Tech-savvy—comfortable using electronic health records (EHRs) and Salesforce.
- Ability to work independently while collaborating with a remote healthcare team.
- Comfortable handling medication-related issues, including prior authorizations and pharmacy coordination.

- Must be available for scheduled calls and virtual meetings as needed.
The Pay Range For This Role Is
30 – 35 USD per hour(USA)
