Position Overview: We are seeking a dedicated and experienced Primary Care Telemedicine
Advanced Practice Provider (APP) to join our team. The APP will be responsible for conducting
follow-up visits with patients after their initial consultations. The primary focus will be on
reviewing lab results, radiology reports, and specialist consultations to ensure ongoing patient
care and treatment plans are effectively managed.


Key Responsibilities:

  • Conduct telemedicine follow-up visits with patients to review lab results, radiology

reports, and consultations from specialists.

  • Provide patient education regarding their health conditions, treatment plans, and any

necessary lifestyle modifications.

  • Collaborate with primary care physicians and other healthcare providers to ensure

comprehensive and coordinated patient care.

  • Document patient interactions, findings, and treatment plans accurately in the electronic

medical record (EMR) system.

  • Prescribe medications and order additional tests or referrals as necessary.
  • Address patient questions and concerns promptly, providing compassionate and patientcentered care.
  • Participate in continuous quality improvement initiatives to enhance telemedicine

services and patient outcomes.

  • Maintain up-to-date knowledge of telemedicine best practices, guidelines, and

regulations.


Qualifications:

  • Current and unrestricted licensure as a Nurse Practitioner (NP) or Physician Assistant

(PA) in the state of Connecticut.

  • Certification as an NP or PA from an accredited program.
  • Minimum of 2 years of clinical experience in primary care, internal medicine, or a related

field.

  • Experience in telemedicine or virtual care is preferred.
  • Strong interpersonal and communication skills, with the ability to build rapport with

patients remotely.

  • Proficiency in using electronic medical record (EMR) systems and telehealth platforms.
  • Ability to work independently and as part of a collaborative healthcare team.
