Job Description:
Works with patient populations identified as “at-risk” in collaboration with their interdisciplinary care team. Identifies and assists in closure of gaps in care related to preventive care, chronic disease care and regulatory services. Assists in delivery of high quality, efficient, and cost-effective healthcare services. Under general administrative direction, gives patient education related to their condition and planned treatment, promote patient self-management, suggests care plans with providers, manages transitions of care, etc. If MSHO Care Coordinator, duties include responsibility for assisting members of members enrolled in a dual eligible Minnesota Health Care Programs to make informed choices about their health care services, conducting assessments, service coordination, and assisting members in making progress towards his or her health goals and coordinating care with the interdisciplinary team. This includes making in-person visit to enrollees’ home.
Licensure/Certification Qualifications:
