Job Title/Position: CARE Center Clinical Concierge – Pinnacle Home Care
JOB SUMMARY
The Care Center Clinical Concierge plays a vital role in delivering seamless, compassionate support to patients, the public, and our employees. The Clinical Concierge provides real-time assistance by triaging incoming calls and communications, ensuring each person’s needs are met promptly and professionally. This role involves accurately documenting all activities, scheduling appointments, conducting follow-up calls, resolving inquiries, and connecting all customers with the appropriate resources and services.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Essential Functions
- Handle incoming and outgoing calls with stakeholders, referral sources, and new patients, delivering exceptional patient care and customer service in a fast-paced, high-volume call center.
- Support clinicians by answering questions related to the Kantime EMR, clinical inquiries, CET processes, and other educational needs.
- Assist patients and clinicians with order processing, critical lab coordination, medical documentation, and other caller requests.
- Efficiently prioritize tasks and manage multiple web-based systems simultaneously, accessing information and performing data entry with high accuracy.
- Adapt smoothly to changes in procedures, programs, or role responsibilities based on organizational needs.
- Ensure HIPAA compliance in all interactions and activities.
- Create, update, and manage confidential client data in the Electronic Medical Record (EMR) system with accuracy and discretion.
- Communicate effectively with both internal and external customers, maintaining professionalism in all interactions.
- Maintain department logs and reports to support internal operations.
- Collaborate with supervisors and leaders to discuss, enhance, and resolve issues effectively.

POSITION QUALIFICATIONS
Required
- Registered Nurse or Licensed Practical Nurse with IV Certification required.
- Exceptional communication and time management skills required.
Preferred
- At least 1 year Home Health Care or telephone triage experience preferred.
