Role Overview
The Registered Nurse, Virtual Care provides high-quality clinical support to patients and caregivers through virtual care platforms. Reporting to the Nurse Cohort Manager, this role is responsible for delivering consistent, patient-centered care, maintaining excellent clinical documentation, and collaborating across care teams to ensure continuity and quality outcomes.
This is a fully remote role designed for nurses who thrive in a fast-paced, mission-driven virtual care environment.

Part-Time- 20–28 (Option for 4, 6, and 8-hour shifts)
Key Responsibilities
Clinical Care & Patient Support
Deliver virtual nursing care using evidence-based clinical judgment and the nursing process
Conduct comprehensive health assessments, triage patient concerns, and provide clinical guidance across multiple care settings
Promote patient health, safety, and wellness through proactive outreach and education
Provide education and clinical support to family and professional caregivers
Communication & Care Coordination
Build strong rapport with patients, caregivers, care coordinators, and partner organizations in a virtual environment
Communicate clearly, compassionately, and professionally with all stakeholders
Identify and escalate urgent or concerning clinical findings according to established protocols
Collaborate with internal care teams to ensure timely follow-up and continuity of care
Documentation & Quality
Maintain accurate, timely, and thorough documentation in the electronic health record (Athena)
Ensure documentation meets PocketRN clinical standards, quality expectations, and regulatory requirements
Adhere strictly to HIPAA and patient privacy standards in all virtual interactions
Professional Practice & Team Engagement
Follow PocketRN workflows, clinical guidelines, and escalation pathways
Actively participate in coaching, performance feedback, and ongoing education
Contribute to continuous improvement initiatives and support a collaborative team culture
Demonstrate accountability for schedule adherence, attendance, and professional conduct
Qualifications
Education
Graduate of an accredited school of nursing
Current BLS certification
Experience
Minimum of 3 years of direct patient care experience
Experience in geriatrics, primary care and dementia care strongly preferred
Experience in telephone triage is preferred
Licensure
Current, active, and unencumbered RN license required
Licensure in all 50 states required

Excellent verbal, written, and asynchronous communication skills
Ability to deliver empathetic, patient-centered care in a virtual setting
Comfortable working independently while remaining engaged with a remote team
Adaptable and eager to learn in a fast-paced health-tech startup environment
Technical & Work Environment Requirements
Proficiency with EHR systems (Athena preferred), Apple/macOS proficiency and Google Workspace tools
