Job Overview

Fairview is looking for an RN Navigator Oncology to join our team! This is a Remote position.

Schedule: This role is casual and remote.

 

Responsibilities: 

Provides care navigation, communication and education to patients and family members through triaging of patients after a referral to cancer care is placed. Works closely with cancer care providers and scheduling team to best meet the needs of patients. Corresponds with referring providers, internal and external as needed. Open lines of communication between RN navigator and patient from referral until initial consultation with cancer care team. The RN Navigator is knowledgeable about treatment options from current literature and standards of care. Anticipates patient needs and takes appropriate action using approved guidelines. Demonstrates excellent problem-solving and communication skills. Assists patients and the care team in navigating any additional work up or testing needed prior to consultation.

  • Navigates patient care from initial referral to oncology until consultation with care team.
  • Serves as point of contact and initial clinical resource to patients new to cancer care.
  • Coordinates care for patients new to cancer care.
  • Collaborates with the multidisciplinary team to develop initial plan of care.
  • Works with patients and families to prioritize patient needs and goals for initial appointments.
  • Anticipates future needs of patient and educates and/or refers to appropriate sources.
  • Assesses patient clinical history and, if known, diagnosis, to create an initial plan for provider appointments and, if appropriate, further diagnostic tests.
  • Collaborates with schedulers to schedule initial appointment(s) and assists scheduling team to resolve any barriers to carry out appointment plan.
  • Provides patient/family education regarding additional testing and work up which is needed.
  • Develops and engages patient and family in plan of care.
  • Works at highest level of scope of practice and skills.
  • Assess patient for any new symptoms from when initial referral was placed.
  • Documents and updates EHR as initial appointment care plan is developed.
  • Tracks appointments and referrals including support services when appropriate.
  • Follows standard work guides and workflows.
  • Collaborates with records team to obtain notes, imaging, results, etc… from outside the health system.
  • Serves as resource to referring offices regarding oncology specialty.
  • Maintains strong communication and relationship with oncology care team and referring office
  • Communicates with referring providers to gain additional clinical history and/or inform of the initial appointment plan. Also communicates recommendations based on cancer care provider review of patient.

Required Qualifications: 

  • Associates Degree in Nursing
  • Or 
  • Bachelors Degree in Nursing
  • And
  • 3 years of adult clinical oncology nursing experience.
  • MN Registered Nurse (RN) License
  • Oncology Certified Nurse within 1-1/2 Yrs
