Remote RPM Alerts Care Coordinator (LPN/LVN)
TimeDoc Health is seeking compassionate and patient-focused LPN to join our RPM Alerts Team and help patients with chronic conditions stay healthier and out of the hospital.
What You’ll Do
- Respond to Remote Patient Monitoring (RPM) alerts and conduct patient outreach
- Support patients with chronic conditions through Chronic Care Management (CCM) and RPM services
- Educate patients about their health conditions and help them achieve their care goals
- Coordinate with providers, office staff, caregivers, and community resources to ensure patients receive the support they need
- Document patient interactions, update care plans, and identify barriers to care
This is a phone-based care coordination role, with team members spending the majority of their day speaking with patients and documenting care.
Qualifications
- Active Multi-State LPN License required
- Minimum 1 year of LPN experience
- Bilingual (English/Spanish) preferred
- Strong communication, documentation, and critical thinking skills
- Care coordination, case management, call center, triage, or geriatric experience is a plus
Schedule Requirements
- 8–29 hours per week
- Some weekdays evenings available
- 3 out of 4 weekends on average
- Flexible 4-hour shift options
- Must work at least 2 major holidays per year
Remote Work Requirements
- Private home office with minimal distractions
- Second monitor required
- Reliable internet with upload speeds of at least 100 Mbps
- Company laptop and software provided
