Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team. Supervises Care Management (CM) staff to include nursing, social work, patient care coordinators and other non-clinical staff to achieve the goals for the organization; collaborates directly with inpatient and ambulatory clinicians and care teams, as well SMG and PCN leaders to support cost-effective high-quality medical services.

Required qualifications:
- Washington Registered Nurse License upon hire
- 5 years of Clinical nursing, including acute care hospital and/or ambulatory clinic experience.
Preferred qualifications:
- Bachelor’s Degree
3 years of Case Management experience including care management, care coordination, discharge planning, or population health.
