Qualifications:

Current, valid, and active nurse compact license to practice as an LPN/LVN in the States of Texas, New Mexico, Colorado, South Carolina and Montana required.
Minimum on one (1) year of Case Management experience preferred;
Clinical nursing experience with demonstrated competence in educating and managing patients with new diagnosis or chronic medical condition.
Experience in clinical settings caring for Chronically Ill Patients; Post-Acute Care experience preferred in Inpatient Rehabilitation and/or Long-Term Acute Care.
Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members and physicians.
Service oriented and able to anticipate recognize and meet the needs of patients remotely, by phone
Able to understand Clinical Pathway Management for patients recently discharged from Post-Acute Care setting.
Knowledge of regulatory standards and compliance requirements.
Strong organizational, prioritizing and analytical skills.
Ability to make independent decisions when circumstances warrant.
Working knowledge of computer and software applications used in job functions.
Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
Knowledge of clinical operations and procedures.
Demonstrates general computer skills including: data entry, word processing, email, and record management.
Effective written and verbal communication skills.
Ability to maintain proper levels of confidentiality.
Ability to maintain quality, safety, and/or infection control standards.

Responsibilities:

The Tele-Clinical Intervention Center Nurse is responsible for responding to clinical needs utilizing the Ernest Health Tele-Center Intervention call center platform. This platform supports patient’s transition to home for 30 days post discharge from a post-acute care setting utilizing a virtual call center approach. Position will be responsible for after hours and weekend Intervention Center Calls.

Posted Total Compensation : $25.00 to $30.00 based on experience. Minimum Salary: USD $25.00/Hr. Maximum Salary: USD $30.00/Hr.
