GENERAL REQUIREMENTS
- Provides direct outpatient primary care to patients by performing diagnostic and therapeutic services to assist physician in diagnosing medical and health and prescribing necessary treatment for primary patient care, records pertinent data for review by physicians.
- Takes patient history; conducts physical exams, records data; writes progress notes and narrative summaries.
- Prescribes medication utilizing clinic formulary for outpatients as feasible and per nurse practitioner protocols.
- Acts as the liaison with medical care resources when referral is needed per protocols
- Requisitions on and off-site lab and x-ray exams as needed per protocol.
- Manages chronic disease utilizing established CommWell Health protocols and procedures.
- Participates in continuing education activities on- and off-site when appropriate and feasible.
- Serves as Mentor/Preceptor for applicable student rotations
- Participates in staff conferences to devise individual patient care plans, to revise and update clinic services, continuing quality improvement program and continuing education as appropriate.
- When there are no patients to be attended to, participates in patient related activities.
- Assist the patient in the development of patient’s self-management goals and care plans. Documents patients progress with meeting goals.
- Focus on assessment, monitoring and proactive management of the healthcare needs of the patient population
- Assist leadership team with forming policies & procedures related to Tele-Health services.
- Provides Tele-Health services based on chief complaint for patients that cannot be placed on their PCP’s schedule due to lace of access.
- Maintain a schedule with patients that are placed on a “sick visit” basis.
- Collaborates with other PCP’s in the organization about the care provided for their patients by communication via “tasks”.
- Collaborates with support staff at all sites when caring for patients in their regions.
- Participates in other related activities affecting the clinic’s programs.
- Participate in PCHH education on the function of self-management tools and techniques.
- Participate in case reviews during PCHH huddles:
o Daily
o Multidisciplinary

BLS Certificate from American Red Cross or American Heart Association

- Full licensure to practice medicine by the NC Board of Nursing or NC Board of Medical Examiners, with valid license.
- National certification as a Nurse Practitioner or Physician Assistant
CREDENTIALING
Requires initial provider credentialing at the time of hire and reappointment every 2 years by credentialing committee.
